Social vulnerability is widely recognized as a structural determinant of population health, yet commonly used national indices may inadequately capture within-region heterogeneity in historically disadvantaged areas such as Central Appalachia. This subregion is characterized by persistent economic distress, elevated disability burden, geographic isolation, and constrained health care access. Standard applications of the CDC/ATSDR Social Vulnerability Index (SVI) rely on nationally derived weighting and ranking approaches that may compress variability within uniformly high-risk regions, thereby limiting their utility for within-region discrimination. This dissertation develops and evaluates a regionally calibrated Central Appalachian Social Vulnerability Index (CASVI) designed to more accurately reflect structural conditions in Central Appalachia.
In Aim 1, we constructed the CASVI by re-weighting and re-ranking SVI component indicators within Central Appalachian counties. Driver-informed weighting strategies were applied to emphasize regional socioeconomic structure, disability prevalence, housing vulnerability, and markers of geographic isolation and access constraints. Rather than relying on national percentile rankings, counties were ranked within the Central Appalachian distribution to generate regionally meaningful quartiles of vulnerability. We examined shifts in county classification under CASVI relative to national SVI rankings to assess the extent to which national metrics masked heterogeneity across Central Appalachian counties.
In Aim 2, we evaluated construct and criterion validity of CASVI using external county-level indicators of structural disadvantage and population health, including all-cause mortality, years of potential life lost, overdose mortality, and broadband access. We compared the discriminatory performance of CASVI with the national SVI within Central Appalachia. The regionally calibrated index demonstrated improved differentiation of counties across multiple domains of structural vulnerability and stronger alignment with regionally salient indicators.
In Aim 3, we applied CASVI to examine county-level adult depression prevalence across 82 Central Appalachian counties as an illustrative population health outcome. Spatial regression models were used to account for geographic clustering and to assess vulnerability gradients within the region.
Collectively, this dissertation demonstrates that regionally calibrated vulnerability measurement improves contextual relevance and discriminatory performance within structurally disadvantaged regions. Findings support the development of place-specific indices to inform targeted public health policy, resource allocation, and intervention strategies in Central Appalachia.

The Institute of Infection, Inflammation and Immunity in Children is excited to announce our 2026 Symposia: ‘From Little Kids to Big Data’. The Symposia will investigate how early life interactions with our microbial neighbors can contribute to important pediatric chronic diseases, such as asthma and food allergy. We have invited a spectacular group of speakers covering a broad variety of topics from microbiology, immunology and mucosal biology that we are certain will be relevant to both UPMC Children’s Hospital researchers and to the greater Pittsburgh science community.

Attendees will have the opportunity to network with the speakers and other potential collaborators. We encourage researchers to submit abstracts to be selected to give a poster presentation. A select group of abstracts will be selected to give ‘elevator pitch’ presentations. We strongly encourage participants to attend in person, though a virtual option will be available. All presenters must attend the symposium in person. The event will include lunch and networking reception/poster session. A detailed schedule will be released closer to the symposium date.

Jenny Blumenthal-Barby, Ph.D., M.A. is the Cullen Professor of Medical Ethics and Associate Director of the Center for Medical Ethics and Health Policy at Baylor College of Medicine.

Dr. Blumenthal-Barby is a philosopher bioethicist whose research focuses primarily on decision-making and ethics.

Dr. Blumenthal-Barby has been the recipient of a Greenwall Faculty Scholar Award in Bioethics, a Pfizer Bioethics Fellowship, and has served as the Principal Investigator on four awards from the Patient Centered Outcomes Research Institute (PCORI) to study and improve decision making in advanced heart failure, one award from AHRQ to study the utilization of an AI algorithm that predicted risk and survival in patients considering LVAD therapy, and two R01s from the NIH Brain Initiative to study neural device ethics and decision making. She has published more than 150 articles in bioethics, philosophy, and medical journals as well as served on several expert panels and committees related to bioethics and medical decision making. She is an Associate Editor for the Journal of Medical Ethics and The American Journal of Bioethics where she also serves on the Editorial Board. Dr. Blumenthal-Barby served as an elected member of the Board of Directors for the American Society for Bioethics and Humanities (ASBH). She has a book out with MIT Press: Good Ethics and Bad Choices: The Relevance of Behavioral Economics for Medical Ethics. In 2023 she was elected a Hastings Center Fellow.

Assessing Healthy Days at Home as a Population Health Indicator in Aging, Serious Illness, and Family Caregiving

Abstract

This dissertation examines person- and family-centered outcomes at the intersection of aging, serious illness, and caregiving, using nationally representative data from the National Health and Aging Trends Study (NHATS), the National Study of Caregiving (NSOC), and linked Medicare claims. Healthy Days at Home (HDAH), a population-level measure of time spent alive and out of institutional care, is gaining traction in quality measurement and value-based payment, yet little is known about whether it reflects the lived experience of older adults and their family caregivers, or how the services designed to maximize home time relate to hospitalization risk.

Aim 1 examines whether HDAH is associated with older adults' self-reported health, depression, anxiety, and participation in meaningful activities. Greater HDAH was linked to better self-rated health, fewer depressive symptoms, and fewer activity limitations, but was not associated with anxiety. Serious illness was independently associated with poorer outcomes across all domains.

Aim 2 extends this analysis to family caregivers, examining how care recipients' HDAH and illness complexity influence caregiver health, emotional well-being, and participation in meaningful activities. Greater HDAH was associated with better caregiver self-rated health but paradoxically higher odds of depressive symptoms. Caregiving intensity, measured by time and task complexity, was a strong predictor of adverse caregiver outcomes, and caregivers of seriously ill older adults experienced greater odds of poor health and activity limitations.

Aim 3 evaluates whether paid personal care services (PCS) and family caregiving intensity reduce hospitalization risk among community-dwelling older adults. Functional dependence, not the receipt or intensity of paid or unpaid care, was the dominant predictor of hospitalization, and living with a spouse or partner was protective in the short term. Neither paid PCS nor family caregiving intensity were independently associated with hospitalization after adjustment. However, PCS recipients with low-intensity family caregiving had persistently elevated hospitalization rates, while those with high-intensity family support did not, suggesting that the effectiveness of paid services may depend on the informal caregiving context in which they are delivered.

Collectively, this work underscores that measures like HDAH should not be interpreted in isolation. Quality frameworks focused on aging in place should pair home time metrics with indicators of functional status, caregiver burden, and illness complexity. This dissertation advances the evidence for person- and family-centered models of home-based care and outcome measurement.

Air pollution in Allegheny County, Pennsylvania — driven by active industrial sources and river valley topography — poses persistent respiratory health risks, yet the pathways linking ambient pollution to personal exposure and respiratory harm remain poorly characterized. This dissertation traces that pathway from outdoor air to indoor environments to the lungs of individuals living closest to emission sources. Outdoor and indoor PM2.5 was simultaneously characterized across 290 homes in 12 municipalities (2016–2021) using Dylos particle counters from the Reducing Outdoor Contaminants in Indoor Spaces (ROCIS) Low Cost Monitoring Project, applying generalized estimating equations and hazard quotient estimation. Using paired indoor-outdoor monitors at the same households, indoor-to-outdoor ratios and household predictors of indoor PM₂.₅ were then examined. Narrowing to the individual level, ten asthma patients from the Asthma and Environmental Health Lung Institute (AEHLI) living upwind and downwind of the U.S. Steel Edgar Thomson Plant were recruited and monitored across five separate two-week monitoring periods (2022–2024). AQMesh multi-pollutant pods and twice-daily spirometry were used to link pollutant exposure directly to lung function.

Across the county, 66.5% of outdoor sites and 10.5% of indoor exceeded the 2024 EPA annual PM₂.₅ standard. Outdoor hazard quotients exceeded 1.0 in 42% of municipalities; indoor HQ values were uniformly lower but approached the concern threshold in communities nearest to industrial sources. Inside the home, outdoor pollution dominated 87.5% of paired monitoring periods (median I/O = 0.318). However, household-level factors — such as air conditioning, frying frequency, candle burning, and pet ownership — significantly modified indoor concentrations. Closer to the source, indoor concentrations near the Edgar Thomson Plant exceeded outdoor levels for PM2.5, CO, and SO2, with significant associations observed between indoor SO2, indoor O3,
outdoor NO2, and reduced FEV1/FVC%. Wall-to-wall carpet emerged as the strongest predictor of uncontrolled asthma (OR = 117.0, 95% CI: 1.94–7,061). Together, these findings reveal the indoor microenvironment as a critical, modifiable link between ambient pollution and respiratory health, supporting expanded hyperlocal monitoring and housing-level interventions in industrially burdened communities.

Using Neural Modeling to Better Understand Speech Disorders

Frank H. Guenther, Ph.D.

Current neuroscientific and computational perspectives on how the human brain plans, initiates, and controls speech will be reviewed. Drawing on neuroimaging, neurophysiology, and modeling research, the talk will highlight how disruptions in these systems contribute to speech and fluency disorders. Clinical implications for assessment, interpretation, and intervention research will be discussed.

Frank Guenther is Professor of Speech, Language, and Hearing Sciences and Biomedical Engineering at Boston University. His research integrates computational modeling with behavioral and neuroimaging methods to study the neural control of speech. He developed the DIVA model, which provides a neurobiologically grounded account of speech production and its disorders.

The Matthews-Rubin Lecture was established to honor Dr. Jack Matthews, who founded the Graduate Program in Audiology and Speech-Language Pathology at the University of Pittsburgh and chaired the Department of Speech from 1955-1987, and Dr. Herb Rubin, who retired in 1991 after 32 years at the University and chaired the Department of Communication and served as Associate Dean of Graduate Studies. The Matthews-Rubin Endowment supports the annual lecture and awards an honorarium to a distinguished
scientist with a national/international reputation in the field of communication science and disorders.

Despite significant positive impacts advancing longevity and protecting health across populations, public health has come under increasing fire. Once well-accepted interventions such as immunizations and fluoridated water, which have saved or improved millions of lives, are now the subject of intense criticism, misinformation, and disinformation. This criticism has been particularly divided along political lines. Such socio-political pressures have increasingly hindered the impact of public health professionals. They simply are not trained to practice public health where it is not welcome. Drawing on real-world examples and through a simple, straight-forward framework, this webinar focuses on ways to empower these practitioners, as well as the academic public health partners who train and support them, to be more effective and impactful in their work. This training will blend didactic and interactive learning approaches to enhance participants’ understanding of these critical skills.

Learning Objectives
At the conclusion of this webinar, participants will be able to:

Vincent X. Liu, MD, MS, is a Senior Research Scientist at the Kaiser Permanente Northern California Division of Research and Chief Data Officer for The Permanente Medical Group. He also directs the Systems Research Initiative, a multidisciplinary team at the Division of Research supported by The Permanente Medical Group (TPMG). His work focuses on the intersection of sepsis, acute severe illness, informatics, and health care delivery with a goal of building towards a learning hospital system. He is a widely recognized expert in applied informatics.

Pregnancy, lactation, and post-partum recovery are as old as time, but insights and recommendations that aim to optimize the health of the mother and the baby are always evolving. This webinar will cover preconception through post-partum, including the needs of both lactating and non-lactating mothers. We will review weight gain recommendations based on pre-pregnancy weight, caloric and nutrient needs, and special conditions like gestational diabetes, as well as the current thinking about seafood, caffeine, and vegan diets. Finally, recognizing that not all those who are pregnant have the same access to healthy foods, we will share resources designed to support nutritional needs.

Learning Objectives
At the conclusion of this webinar, participants will be able to:

Adverse Childhood Experiences (ACEs), chronic stress, and secondary stress can affect how individuals regulate emotions, process information, and engage in conversations—particularly during challenging or high-stress interactions. Drawing on the Neurosequential Model developed by Dr. Bruce Perry, this training introduces bottom-up brain strategies that support regulation before attempting problem solving or difficult conversations. Participants will explore the Regulate–Relate–Reason sequence and learn why regulation and connection often need to come before reasoning or problem-solving. Through guided reflection and break-out discussions, participants will assess their own stress triggers and responses, consider how dysregulation can influence interactions with clients and colleagues, and identify practical strategies to manage stressful encounters.

Learning Objectives
At the conclusion of this webinar, participants will be able to:

Professionals working in public health, health care, and community settings often encounter high levels of stress—both in the communities they serve and within their own work environments. Experiences such as Adverse Childhood Experiences (ACEs), chronic stress, and secondary stress can influence how people interpret interactions, respond, and navigate difficult conversations. In high-pressure settings, communication can quickly shift toward conflict, withdrawal, or self-blame. This training examines how stress and moments of perceived criticism, disconnection, or misunderstanding can trigger defensive responses that shape how we communicate and relate to others. Participants will examine common responses that arise during challenging interactions and consider how these responses may affect themselves and others. The session will introduce trauma-responsive communication practices that help reduce defensiveness, increase understanding, and support more constructive conversations.

Learning Objectives
At the conclusion of this webinar, participants will be able to:

This is the 3rd workshop of our four-part workshop series “Mastering Single-Cell Analysis with Partek Flow” that will provide in-depth exploration of single cell technologies and their applications. Leveraging Partek Flow software, this series will cover the entire workflow, from data import, pre-processing to results interpretation and visualization.

Workshop 3:  Advanced ScRNASeq Analysis Using Partek Flow focusing on Multiple samples dive into an advanced ScRNASeq analysis techniques using Partek Flow, exploring multiple sample datasets. Enhance your skills in complex single cell rnaseq data analysis covering clustering options, batch effects correction methods, and visualization techniques.

Target Audience: Experimental biologists seeking to analyze single cell data generated through experiments or retrieved from public databases. The software covered in the workshop operates through a user-friendly, point-and-click graphical user interface, so programming experience is not required.

This workshop offers an introduction to analyze and visualize the gene expression data through network-based approaches. Participants will learn how to upload gene lists to public protein interaction resources such as STRING DB to retrieve relevant interaction networks and import them to Cytoscape. Then the attendees will explore methods for analyzing gene ontology terms and biological pathways within the context of the network. The workshop utilizes the free, user-friendly Cytoscape software, which features a graphical, point-and-click interface suitable for researchers at any experience level

Target Audience

Experimental biologists seeking to learn how to visualize the data generated through experiments or retrieved from a publication or a repository such as GEO. The software covered in the workshop operates through a user-friendly, point-and-click graphical user interface, so neither computer programming experience nor familiarity with command line interface is required.

Upon completing this class, you should be able to:

Upload gene lists to public protein interaction databases (e.g., STRING DB) and retrieve interaction networks relevant to their data.Import and visualize these networks in Cytoscape, mapping gene expression values to network nodes using customizable visual styles.Analyze and interpret functional associations such as gene ontology enrichment and biological pathways witin the network context.Utilize various apps and apply network layouts to highlight biologically meaningful patterns in their data.Export publication-ready network visualizations and results for further analysis or sharingDate: July 8, 2026

This advanced class presupposes knowledge of basic PubMed searching.

The class will provide an overview on using HSLS and NLM resources. The class will discuss database searching for drug information, as well as uses for drug encyclopedias such as Micromedex to search for specific properties of drugs. Resources for clinical and research use of drugs will be included. Sources for Drug Evidence Alerts, calculations and interactions will also be covered.

Are you interested in using generative AI tools but are concerned about lack of data transparency and/or privacy? In this class, we will talk about different options for generative AI tools and how your choice of tool affects your privacy and the transparency of how said tool will use your data. You'll learn to navigate privacy policies, compare data practices, and evaluate user control options. At the end of the class, you'll be equipped to make informed decisions about which AI tools align with your privacy and transparency values.

Upon completing this class, you should be able to:
